Overview
Chronic Care Staffing (CCS) is seeking a compassionate and detail-oriented Remote Care Coordinator (CMA/RMA) to join our chronic care management team. This role is ideal for certified medical assistants who thrive in a virtual care environment and enjoy supporting patients managing chronic conditions.
As a Care Coordinator, you will serve as a consistent point of contact for patients, providing outreach, education, care coordination, and documentation — all within a HIPAA-compliant remote setting.
Key Responsibilities
- Conduct monthly Chronic Care Management (CCM) outreach calls and verbal enrollments
- Perform health risk assessments and assist with Transitional Care Management services
- Support Remote Patient Monitoring (RPM) programs and chronic condition tracking
- Communicate with patients and families regarding medications, diagnoses, and care plans
- Collaborate with physicians and healthcare teams to ensure coordinated care delivery
- Promote adherence to individualized care plans and patient education goals
- Connect patients with appropriate community resources when needed
- Maintain accurate documentation in EHR systems and meet call volume expectations
- Ensure compliance with CMS guidelines and CCS quality standards
Required Qualifications
- Must live in PST or MST time zone
- Active, unencumbered CMA or RMA certification
- Current BLS certification
- Recent experience in a clinical, care coordination, or CCM setting
- High proficiency in EHR systems
- Proficiency in Google Suite
- Strong clinical reasoning, critical thinking, and problem-solving skills
- Excellent verbal and written communication skills
- Ability to work independently in a HIPAA-compliant home office environment
Preferred Qualifications (Nice to Have)
- Prior experience in care coordination, chronic care management, transitional care, or remote patient monitoring
- Familiarity with community resource navigation
- Experience educating patients and families about chronic conditions and medications
- Background in quality measures, reporting, or CMS compliance
- Demonstrated ability to succeed in a remote team-based environment
- Additional coursework or certification related to care or case management
